ABSTRACT

Objective:To investigate the effects of comprehensive geriatric assessment(CGA)and intervention on treatment outcomes of type 2 diabetes and concurrent emotional disorders in the elderly.Methods:108 type 2 diabetes cases with emotional disorders were enrolled from March 2020 to March 2021 in the First Affiliated Hospital of Zhengzhou University and randomly divided into the conventional treatment group and the CGA group(54 cases each). The control group was given standard drug treatment and psychological counseling, whereas the CGA group was additionally given individualized treatment based on CGA guidelines.After observation for 8 weeks, clinical effects in the two groups were examined.Changes in cognitive status, nutritional status, fall risk and results on other indicators in patients were comprehensively assessed and statistically analyzed.Results:Compared with pre-treatment data, 8 weeks of treatment for both the control group and the CGA group resulted in decreased FPG levels[control group(10.16±0.90)mmol/L, (8.70±2.98)mmol/L, t=2.58, P=0.011; CGA group: (9.94±0.82)mmol/L, (7.12±2.25)mmol/L, t=8.65, P=0.000], 2hPG levels[control group: (11.83±3.92)mmol/L, (10.53±2.70)mmol/L, t=2.01, P=0.047; CGA group: (10.64±2.99)mmol/L, (9.26±1.89)mmol/L, t=2.87, P=0.005], HbA 1C(%)[control group: (9.76±1.09)%, (8.66±2.64)%, t=2.83, P=0.006; CGA group: (9.38±2.92)%, (7.81±1.78)%, t=3.37, P=0.001], HAMD-24 scores[control group(25.69±4.70), (24.20±2.48), t=2.06, P=0.042; CGA group(24.03±4.00), (22.11±1.38), t=3.33, P=0.001], and HAMD-14 scores[control group(19.66±2.84), (18.41±2.34), t=2.50, P=0.014; CGA group(18.77±2.48), (17.39±2.36), t=2.96, P=0.004], respectively, with more marked changes in all measures in the CGA group than in the control group(all P<0.05). Conclusions:Early CGA can effectively improve blood glucose levels and relieve mood disorders in elderly type 2 diabetes with emotional disorders.

ABSTRACT

Objective To investigate the clinical effect of mosapride combined acupoint massage on diabetic gastroparesis(DGP)in elderly patients with type 2 diabetes.Methods 150 cases of DGP in our hospital from April 2015 to May 2016 were collected and randomly divided into three groups:the conventional treatment group(n=35,in insulin treatment),the control group(n=52,in mosapride treatment),and the experimental group(n=63,in mosapride combined with acupoint massage treatment).All patients received the conventional diabetic therapy.Clinical efficacy,gastric emptying rate and the levels of serum gastrin and fasting blood glucose were observed and compared before versus after treatment among the three groups.Results After treatment,the total effective rate was 92.1% in the experimental group,versus 51.4% and 73.1% in the conventional treatment group and control group respectively,showing a statistically significant difference(P<0.05).There were significant differences in gastric emptying rate,the levels of serum gastrin and fasting blood glucose among the 3 treatment groups before versus after treatment.And gastric emptying rate,the levels of serum gastrin and fasting blood glucose were improved in the experimental groups compared with the conventional treatment and control groups(all P<0.05).The follow-up results showed that the adverse reactions and the recurrence rates were significantly lower in the experimental group than in conventional treatment group and control group(all P<0.05).Conclusions Mosapride combined with acupoint massage in the treatment of diabetic gastroparesis have significant clinical effect,low recurrence rate and high safety,and is worthy of promotion.

ABSTRACT

This study was aimed to explore effects of Jinzhen Koufuye (JZKFY) on acute lung injury (ALI) induced by lipopolysaccharide (LPS) in rats. Rats were randomly divided into the normal group, model group, positive control group, and three JZKFY groups with the drug dosage of 4.4, 2.2, 1.1 g·kg-1, respectively. Intragastric administration was given to animals for 7 consecutive days. One hour after the last administration, ALI model was induced by the injection of LPS (6 mg·kg-1), and then rats were killed at 16 h after saline (control) or LPS injection. Histological examinations were performed on the lungs. At the same time, the lung permeability index, activities of MPO, MDA and SOD in lung tissues, contents of TNF-α, IL-6 and IL-1β in serum were measured, respectively. The results showed that JZKFY of high-, medium-, or low-dosage can significantly reduce JZKFY lung permeability index. JZKFY of high- and medium-dosage can obviously regulate activities of MPO, MDA and SOD in lung tissues, re-duce contents of TNF-α, IL-6 and IL-1β in serum, as well as significantly improve lung tissue lesions in ALI. It was concluded that JZKFY might improve ALI induced by LPS through inhibiting lung vascular permeability, reduc-ing the lung neutrophil aggregation, improving the ability of antioxidative stress and lowering inflammatory response.

ABSTRACT

Objective To analyze the diversity of intestinal microecology in different healthy age groups and explore the relationship between age and the composition of intestinal microflora.MethodsFecal samples were collected from the children,adult and elderly groups.The main intestinal microflora were cultured and quantified,and the qualitative and quantitative changes in the level of category were determined by fluorescence quantitative PCR.The Bifidobacteria/Enterobacteriaceae (B/E) ratio reflexing intestinal colonization resistance was also calculated.Results The germiculture of fecal samples indicated that the counts of Bifidobacteria and Lactobacillus (P<0.01 and P<0.05,respectively) were significantly lower in the adult and aged groups than in children group.The number of Bacteriodes and Enterobacteriaceae in the elderly group were higher than in the children and the adult groups (all P<0.01).The counts of Enterococcus were increased in the adult and the aged groups (P<0.05 and P<0.01) compared with the children group,and were higher in the aged group than in the adult group (P<0.05).The B/E ratio was significantly decreased in the aged group (0.88-±-0.13) as compared with the children (1.15±0.15) and the adult groups ( 1.01± 0.20) ( both P < 0.01).The diversities of Bifidobacteria,Bacteriodes,Enterobacteriaceae and B/E ratio by fluorescent quantification PCR were consistent with the results by germiculture technology.The diversity of Lactobacillus (P<0.01) between the children and the aged groups as well as between the children group and the adult group significantly improved (P<0.01 and P<0.05 ) in PCR compared with the germiculture technology. ConclusionsThe numbers of intestinal Bifidobacterium and Lactobacillus are decreased with lower colonization resistance,while the numbers of Bacteriodes,Enterobacteriaceae and Enterococcus are increased with aging.
